If you’ve ever had “your testosterone” checked and been handed a single number, you got a partial answer. A real testosterone workup measures free testosterone, estradiol, LH, FSH, and prolactin — plus baseline bloodwork like a CBC to watch your hematocrit — because those numbers together tell a provider why your levels are what they are, and whether TRT is even the right move. Here’s what each one actually does, and why a medical provider licensed in your state reads the whole panel, not one line.
Total testosterone is the headline, not the story
Total testosterone is the number most men know — and it’s a starting point, not a diagnosis. It’s best measured with a morning blood draw, because testosterone levels are typically highest in the early morning.[1]
And because a single value can be thrown off by illness, stress, or timing, providers often confirm low testosterone with two separate low readings before diagnosing it (hypogonadism).[2] One number on one morning isn’t enough to start therapy.
Free testosterone: the part that’s actually available
Most of your testosterone is bound to proteins in the blood and isn’t biologically active. Free testosterone is the fraction your body can actually use.[1] Two men can have similar total testosterone but very different free testosterone, which is why measuring (or calculating) it matters. It often explains symptoms that “normal total” numbers don’t.
LH and FSH: pointing to the cause
Luteinizing hormone (LH) and follicle-stimulating hormone (FSH) are signals from your brain’s pituitary telling your testicles to produce testosterone and sperm. Their pattern helps a provider locate where the problem is:
- Low testosterone with high LH/FSH points toward a testicular cause (primary hypogonadism).[3]
- Low testosterone with low or normal LH/FSH points toward a pituitary/hypothalamic cause (secondary hypogonadism).[3]
That distinction changes what a provider recommends — sometimes it means the answer isn’t TRT at all, but addressing an underlying cause. This is exactly the reasoning a “just check my T” service skips.
Estradiol and prolactin: the supporting cast
Estradiol (a form of estrogen) matters in men too — testosterone converts to it, and too much can drive problems such as gynecomastia, erectile dysfunction, or infertility.[4] It’s worth a baseline and gets monitored on therapy.
Prolactin is checked because an elevated prolactin level can itself suppress testosterone,[5] and occasionally signals something a provider needs to investigate before starting treatment.
The safety baselines: CBC, hematocrit, and PSA
Two of the most important numbers on the panel aren’t hormones:
- CBC / hematocrit. Testosterone therapy can raise red blood cell count. A baseline lets your provider watch for that on treatment, because too-high hematocrit is a real risk that needs managing.[2]
- PSA (prostate-specific antigen), for men in the relevant age range, is a baseline before starting therapy and something a provider tracks.
These aren’t box-checking. They’re the difference between managed therapy and a gamble.
Why the whole panel, and why it repeats
Reading all of this together is the actual medical work — the part marketing glosses over. It’s how a provider decides whether your levels are genuinely low or just borderline, whether your symptoms line up with the numbers, and whether there’s a reason testosterone would be a bad idea for you right now (a history of prostate or breast cancer, untreated severe sleep apnea, elevated hematocrit, uncontrolled heart failure, or plans to father children soon — since TRT can suppress fertility).[2]
And it doesn’t stop at baseline. If you start TRT, you’ll have follow-up bloodwork — commonly within the first few months and then periodically — to confirm your levels land in a healthy range and to monitor hematocrit, estradiol, and how you’re actually doing.[2] TRT is a relationship with your labs, not a one-time transaction.
